Better research some more. You’ll find that TESE only works with IVF. Why? Sperm that is extracted is "immature" sperm and it can’t swim or penetrate an egg to fertilize it on it’s own. Extracted sperm can only be used for IVF, which you are right, is expensive and doesn’t always work. You can check my source below and read up about it. Your cheapest option would be a reversal, but if he’s not having one, then you’re looking at IVF or IUI with donor sperm. Btw, I’m a reversal wife and mother.
